APPLICATION FORM FOR THE EXERCISE OF PERSONAL DATA PROTECTION RIGHTS OF CEDIA

Procedure for handling requests to exercise rights

The request to exercise rights submitted by the Owner of the personal data or their Legal Representative will be attended to by CEDIA within a maximum period of fifteen (15) days, counted from its receipt.

Within five (5) days, CEDIA may require the Data Subject or their Legal Representative to clarify or complete the information provided in the application, through a formal notification. In this case, the Data Subject or their Legal Representative will have a period of ten (10) days to comply with said requirement.

If no response is received within the specified timeframe, CEDIA may close the request with a duly justified notification. This does not limit or affect the right of the Data Subject or their Legal Representative to submit a new request to exercise their rights at any time.

The official channels enabled for receiving and submitting requests to exercise rights are the following:

  2. Submission of the corresponding form in PDF format, duly signed electronically, attaching a copy of the identity card or passport and necessary attachments
  3. CEDIA 's headquarters , located at Gonzalo Cordero Dávila 2-122 and José Fajardo .

In accordance with the provisions of the Organic Law on the Protection of Personal Data, you are hereby informed that the personal data provided through this form will be processed by the data controller for the sole purpose of managing and processing your request to exercise your rights. Your personal data will not be disclosed to third parties.
